Telehealth Informed Consent

This page summarizes what telehealth is and what you should understand before receiving care through LIFE NP. You will review and sign a formal informed consent document during patient intake; this summary is provided for transparency.

What telehealth is

Telehealth is the delivery of healthcare services using electronic communications, such as secure video visits, between a patient and a provider in different locations. Your LIFE NP provider will use telehealth to evaluate, diagnose, and treat you where clinically appropriate.

Benefits

  • Access to care from home or work, without travel or waiting rooms.
  • Easier ongoing follow-up and monitoring.
  • Reduced exposure to communicable illness.

Limitations and risks

  • Telehealth does not allow hands-on physical examination. Your provider may determine an in-person evaluation with a local clinician is needed before or during treatment.
  • Technology can fail. If a video visit is interrupted, we will reconnect or reschedule promptly.
  • As with any electronic communication, security protections cannot be absolutely guaranteed, although we use secure, encrypted systems for clinical care.
  • Telehealth is not appropriate for emergencies. If you are experiencing a medical emergency, call 911.

Your rights

  • You may withhold or withdraw consent to telehealth at any time without affecting your right to future care.
  • You may ask questions about any aspect of your evaluation or treatment, including alternatives and costs, at any time.
  • Your health information is protected under applicable state and federal law, and our Notice of Privacy Practices is provided during intake.

Location requirement

State law requires that you be physically located in a state where your provider is licensed at the time of each telehealth visit. LIFE NP currently serves patients located in Arizona and North Dakota.
